After you get the form, you need to fill it exactly as asked. I have had to do it twice.

Here are some questions asked and if it is your record, they have to be accurate.

Some records were destroyed July 1973 at the National Records Center. These records stored and suffered the most damage were:

Army veterans discharged or deceased between Nov 1, 1912 and Dec 31, 1959.

Air Force veterans deceased Nov 1, 1964 and Dec 31, 1959.

Air force veterans discharged, deceased or retired Nov 1, 1912 and Jan 1, 1964, whose names come alphabetically after James E. Hubbard.

Questions are: Where you were born, parents, date of birth Ss, training or active duty, where you enlisted at and date, name of close relatives when you enlisted, place of induction, where you did basic training, type of military service. separation station, month and year of separation, file claim of VA benefits, receive a bonus for military service, serve in national guard, retire from military service, spend time disability list, and did you work for federal government?

My advice is to get all your papers in order. Then when papers come, all you do is add entries.
