West Union grave marker mystery

My GG-Aunt, Isabel Sanford, is buried in West Union Cemetery. The information on the grave marker is exactly as expected EXCEPT there is an additional two lines of inscription at the very bottom. It appears to say: "P.E. Hm________, D _________."

Does the cemetery have records other than the markers that might explain this? Any ideas what this could be?
