Presbyterian Hospital

My ggfather Charles Hennessey died in 1915 (age 27) of peritonitis (emergency appendectomy) at 'Presbyterian Hospital' and is buried at 'Woodlawn Cemetery'.

This is the story which has come down through the family. I grew up in Waterloo and have never heard of Presbyterian Hospital or Woodlawn Cemetery. Does anyone out there have information on either of these?

Also, How do I contact BlackHawk County for a death cert, and how much is it? I must do this long-distance as I live in OK.
